GENERAL MEDICAL & SPECIALISED INPATIENT BEDS

GP Beds
GP Beds are primary care inpatient services for an acute episode of illness for people that cannot be managed at home. Expected maximum length of stay is 14 days. The service is available for patients of all ages, although because children's conditions can deteriorate rapidly it is unlikely that they would be referred to this service.

Convalescent Care
This is a similar service to that of Transitional Care, however, it does not have the same age criteria and is funded from a different source at the District Health Board. Referral is directly from the medical wards at Waikato Hospital and a Waikato Hospital order number needs to be provided before we can accept the referral.

Private Care
Pohlen Hospital can provide private hospital care for those who do not meet the necessary DSL criteria for funded beds or who wish to extend their stay after the funder's of their service has deemed them well enough to be discharged. Please talk to us about your needs.

RESPITE & AGED CARE

High Quality Care
Pohlen Hospital has a reputation for providing high quality, hospital level continuing care for older members of our community.
Whilst we are not a rest home, we maintain our certification to provide rest home care in order to care for couples with different levels of assessment or for those people being assessed for the first time and who may wish to transfer to a rest home after assessment.
Assessments are performed by Disability Support Link (DSL), which is the Needs Assessment and Service Co-Ordination Agency for the Waikato.

Respite Care
These beds are for the short term care of those clients who are cared for in the community by a full-time career. Respite care is allocated a number of days per year – usually 28. If you require more advice please see your GP.

TRANSITIONAL CARE

Care for patients over 65
Transitional care is provided to patients over 65-years-old to allow an early discharge from an acute ward at Waikato Hospital following a severe medical or orthopaedic event.
Admission to the service is via assessment and referral from the Waikato Hospital Transitional Care Team. The service is designed to facilitate the transition from hospital to home and/or from medical dependence to functional independence.

END OF LIFE CARE

The emphasis on care provided during the final weeks of life is focused on the dignity, comfort and the overall care of families and loved ones.
Clients and their families are provided with individual spacious rooms which include foldaway wall-beds for the use of family and whānau.
Referral for this service requires assessment and approval by DSL.

OUTDOOR EXERCISE & REHAB

Free Facility for Locals
This facility is going from strength to strength since opening in 2010
The facility and associated programme is free to use by any adult in our community. This is definitely the key to its success as cost and accessibility are the two main barriers for people wanting to exercise.
The Outdoor Exercise and Rehabilitation Facility is well utilised by a wide range of people. Their needs include:

  • Strength and balance

  • Fitness

  • Coordination

  • Rehabilitation

  • Weight loss

  • Meeting new people

COMMUNITY SERVICE

Our community care helps people stay well at home and connected to local support. Services include Meals on Wheels, district nursing, and equipment hire all designed to support independence, recovery, and everyday wellbeing across Matamata. 

Meals on Wheels
Meals on Wheels is a Volunteer service whereby members of the community use their own transport to deliver lunchtime meals. The cost is $14.00 for a main meal and $18.00 for a main meal and dessert.
Anyone in the Matamata Township can receive Meals on Wheels. Call Pohlen Hospital for more information on 07 8819100 ext 9
